NEW INSURANCE FORM SOON REQUIRED —
- The CMS 1500 form has replaced the HCFA 1500 form.
The National Uniform Claim Committee (NUCC) has revised the 1500 form. The new form, the CMS-1500 (08-05), will replace the current HCFA 1500 form. Insurance carriers will soon discontinue acceptance of the HCFA 1500 form and accept only the CMS-1500 (08-05). The claim form has been updated to accommodate the mandated National Provider Identifiers (NPIs). Additional changes were also made in area 24 of the form. ShrinkRapt 6.0 completes all areas of the new form. The CMS 1500 forms and envelopes may also be ordered directly from Saner Software.

- NPI numbers
Box 17b, 24j, 32a, and 33a can be completed with NPI numbers. Click the NPI number link on the link to learn more these numbers. 
- Non-NPI #s and Qualifiers
In additional to NPI numbers, an insurance carrier may assign a non-NPI number and qualifer to a referral source, service facility, rendering provider and billing provider.
- Additional changes in area 24
In area 24, service dates have been reformatted, area c has changed as well as I and J. Twenty four K has been removed.
