Box 17 of the claim form may require a referral source NPI number and a non-NPI number and qualifier.

- Rendering Provider
ShrinkRapt prints the NPI number and non-NPI number and qualifer (when required) of the redering provider on the new CMS 1500 in box 24J.
- ServiceFacility
Each place of service may be assigned id#s that are to be printed in box 32a and 32b of the claim form.
- BillingProvider
Your NPI number must be printed in box 33a of the claim form. A non-NPI qualifier and number may also be required in box 33b.
